Yes, you can make an appointment at more than one Crossover Health Center. While it’s encouraged, you are not required to establish care at one location.
Yes, Meta employees who work in San Francisco can access Crossover Health’s network of Centers in the Bay Area.
Crossover has partnered with Lyft to transport you and/or your qualified dependents to and from a Crossover location. You will receive a Lyft voucher code in your appointment reminder (or confirmation email if booked <24 hours before their appointment) that the rider must enter into their LYFT app in order to apply towards the ride. The ride must either stop or start at a Crossover location in order to be considered a valid ride.

No. But adult dependents (18+) can access one of Crossover Health’s five Bay Area locations.
Interns are automatically enrolled into Meta’s EPO medical plan and have 31 days to decline the provided insurance. Currently, Interns do not have access to the onsite Life@ Wellness Center, but can still access the other Crossover Health locations.
The Wellness Centers and Crossover Health are able to verify your eligibility as part of the sign up / sign in process.

By setting up an online account you will be able to schedule an appointment, message your provider, view test results, and request a refill on your medication. More importantly, establishing an account is the first step in creating a trusted relationship with a Life@ Wellness Centers provider who can then be available to provide assistance 24/7 for serious health issues. Kaiser members will also want to sign up here to message providers they are seeing at our Health Centers including acupuncturists, and chiropractors. Currently, there are no Kaiser Primary Care Physicians onsite, so please visit kp.org for care.
Your personal health information is protected by federal privacy and security regulations. Crossover Health complies with HITRUST to protect personal health information, following standards for privacy, security, and network architecture. As a separate Medical Group, we do not share identifiable health information with your employer. Any violation of this privacy policy by Meta or any other person is a clear violation of HIPAA legislation and carries significant legal and financial consequences.
After creating your account at facebook.crossoverhealth.com, you can log in to schedule an appointment. Select the appointment type, the location, the provider, and the date and time you would like to be seen. You will get a confirmation email regarding your appointment.

There are multiple avenues to provide feedback to the Wellness Centers including: completing a Patient Satisfaction Survey after your appointment, calling (650) 521-5440, or talking directly to a Practice Manager on-site.
If you need care outside our Wellness Centers, our care team will help coordinate a referral. We also work with the referring specialist or imaging facility to ensure that all the information from outside care is collected back into your medical record for complete documentation.
The price of a visit depends on your benefit design and insurance. This typically includes a nominal co-pay, deductible or coinsurance for services. Our hosts will share the fee schedule with you upon request.

We accept credit cards and debit cards. If you are enrolled in the Meta Sponsored HDHP insurance, we also accept cards associated with your HSA. Please note we no longer accept cash payment.
Under the HDHP plan, you will pay the full cost of all non-preventive services until your deductible is met. Once you’ve met your deductible, you will be responsible for 10% of the costs related to each of your visits (also known as coinsurance). You will be required to pay the deductible or coinsurance amount by credit, debt or using your HSA card when you check in for your appointment.
Standard co-pay rates apply for both medications ($5 for generic brand and $35 for name brand). Rates may vary based on insurance coverage.
We selected Crossover Health Medical Group as our partner to manage the Life@ Wellness Centers. You may also see a Kaiser physician at MPK49, however, these services have been temporarily suspended.

Crossover Health provides comprehensive primary care. They employ board certified primary care physicians, and other licensed professionals like registered nurse, physical therapists, chiropractors, psychologists, licensed marriage family therapists, acupuncturists, optometrists, dietitians, and health coaches.

Advance directives are legal documents that allow you to spell out your decisions about end-of-life care ahead of time. They give you a way to tell your wishes to family, friends, and healthcare professionals.

A living will takes effect when you are terminally ill and unable to express your wishes about your care. It documents the type of treatment you want, but it does not let you designate someone to make decisions for you (a healthcare power of attorney does that). With a living will, you can instruct your family and doctors on your wishes concerning life support.

The Wellness Centers can dispense medication at the time of your visit. Normal co-pays for the medication will apply. For any medication not carried in the Wellness Centers, the physicians can electronically send the prescription to the pharmacy of your choice or to Alto for Wellness Center or home delivery.
The health information saved in our electronic health record is accessible through several export standards utilized throughout the health industry. The EHR used by Crossover Health takes advantage of the Continuity of Care Record (CCR) format. This is accessible upon request and within your online account.
